Idiot-Proofing, the higher end technique
- As maintenance engineers, my team had to analyze every failure in machines, find the root cause of the failure and take preventive actions to avoid recurrence of such failures in all likely places. In spite of taking all Fool-Proofing steps, we sometimes faced recurrence of similar failures. Obviously Fool -Proofing was not enough. Then I coined the higher end technique "Idiot-Proofing". We had to make the systems involving people, machines and processes, to function well, so that even idiots can not make mistakes.
After many years of retirement, I had
to apply this technique on myself, when one day, I filled the gas tank
in my car and drove away leaving the gas tank cap at the gas
station. After driving to the destination, I realized my mistake.
Luckily, I went back to the gas station and found the cap on top
of the filling panel. Then I decided I must Idiot-Proof this
recurring operation.
From that day, I adopted the practice of placing the gas cap in the middle of the driver seat, upside down, immediately after opening it. What would happen when I forget to replace the cap? I would be sitting on it,as I try to drive off and get automatically reminded! The process is now Idiot-Proof!
